World’s first baby born via womb transplant from dead donor

The mother was a 32-year-old woman with Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome - a condition that causes female sexual reproductive organs, such as the uterus and vagina, to be underdeveloped or absent at birth.

The girl was healthy weighing almost six pounds and was delivered via cesarean section.

"The results establish proof-of-concept for treating uterine infertility by transplantation from a deceased donor, opening a path to healthy pregnancy for all women with uterine factor infertility, without need of living donors or live donor surgery", Brazilian researchers said in the study published Tuesday in the medical journal The Lancet. "However, the need for a live donor is a major limitation as donors are rare, typically being willing and eligible family members or close friends". Baylor has had two successful births from live donations; Cleveland Clinic is working toward deceased donations; and her own program will be performing both living and deceased donor transplants over the next year.

In total, there have been 39 procedures of this kind, resulting in 11 live births so far.

In 2016, doctors at the Cleveland Clinic in OH transplanted a uterus from a deceased donor, but it failed after an infection developed.

It said the case involved connecting veins from the donor's uterus with the recipient's veins, as well as linking arteries, ligaments, and vaginal canals. In Sweden in 2014, doctors for the time time helped a woman with a transplanted uterus give birth; since then, there have been about a dozen such babies born around the world.

In a landmark move, a baby in Brazil has been born to a woman with a transplanted uterus from a deceased donor.

Part of the challenge in transplanting a uterus from a deceased donor is that the process - obtaining an organ, matching it to a recipient based on blood type and other qualities, and completing the operation - can take time.

Professor Lois Salamonsen, research group head of endometrial remodelling and Adjunct Professor at Monash University's Department of Obstetrics and Gynaecology said she had received a few calls form Australian women over the years asking why uterine transplantation wasn't done in Australia.

After the recipient's womb and ovarian are functioning, the embryos are transplanted into her uterus - generally at least one year after the transplant.

Doctors implanted the fertilized eggs seven months after the procedure.

It is hoped that this, the first ever uterine transplant involving a deceased donor and the first uterine transplantation in Latin America, could enable more women struggling with fertility issues to undergo treatment without having to wait for a live donor. Doctors were able to remove eggs, fertilise them with the father-to-be's sperm and freeze them. The drugs have triggered pregnancy complications in some of the Swedish patients, including kidney problems and preeclampsia, she notes.

The baby girl was seven months and 20 days old, weighing 7.5kg when the researchers wrote their report. A new technique might broaden the pool of available uterus donors and give more women the option to have a baby.
